What is a FHO ?

A Family Health Organization (FHO) is an initiative of the Ontario Ministry of Health and Long-Term Care. A FHO is a group of family physicians who are working together to give you and your family better access to quality primary health care services.

Can I join your FHO ?
Can I go to another walk-in clinics if I am part of your FHO?

We recommend that you DO NOT go to walk-in clinics outside of Dixie Road Medical Associates FHO.

If you belong to the Dixie Road Medical Associates FHO and you use a walk-in, our office will be docked the cost of your walk-in visit.  Please contact us if you require an urgent same-day appointment.

Additionally, if you go to a different walk-in clinic, that clinic DOES NOT FORWARD YOUR RECORDS to DRMA, so our records of your health will not be complete.

The government compensates Dixie Road Medical Associates FHO for remaining open extra hours on the weekend and into the evening all weekdays to ensure you have the access to urgent medical care that you need.  If you need help outside of our hours, we recommend that you use the Urgent Care or Emergency Department services of our home hospital, Trillium Health Partners.  If you tell the hospital who your family doctor is, we will be notified of the care you received in hospital.

What services are not covered by OHIP for patients that are part of a FHO?

The Ontario Health Insurance Plan (OHIP) pays for most of your medical visits to our office.

There are, however, uninsured services which OHIP does NOT COVER, such as:

  • medical forms for insurance
  • forms for legal or employment purposes
  • Driver and Marine Medicals
  • photocopying, faxing and transferring medical records
Is there a fee for Sick Notes?

The Ontario Health Insurance Plan (OHIP) pays for most of your medical visits to our office.

There are, however, uninsured services, which OHIP does NOT COVER, such as sick notes and medical fit notes for work. Please contact our Medical office assistants or Business manager for further information.

Do you accept private insurance?

Yes. Some of the services we provide that are uninsured by OHIP may be covered by your private insurance plans. Contact your insurance provider to find out what level of coverage you have. We are happy to provide the necessary invoicing for insurance claims. However, we have no direct dealings with insurance companies and an invoice will be provided once a payment has been made.

What types of payment are accepted?

We only accept Visa, MasterCard, Interac and cheques for appointment payments. If you do not use or have access to these payment methods, please contact our clinic to discuss alternate options. Unfortunately, we do not accept American Express.

What is a same day appointment

A limited number of these appointments are available on a first-come, first-served basis with one of our doctors for appointments that are urgent and cannot wait for a regular booked appointment.

Examples of this include:

  • urinary tract infections
  • throat infection
  • respiratory tract infections
  • new uncontrolled pain

A periodic health heath exam (routine physical) cannot be booked as a same day appointment.

VISITING THE CLINIC

Where are you located?

Dixie Road Medical Associates is conveniently located at 2200 Dixie Road, just south of the main intersection of Dixie Road and Queensway. You can access it by buses No. 4 and No.5.

What are your hours?

We operate between 8:00AM and 8:00PM on weekdays and 10:00AM – 3:00PM on Saturdays.  We are closed on Sundays and on statutory holidays.

Do you have parking?

We have over 25 free parking spaces on our premises.

Do you have an accessible entrance ?

Our wheelchair/walker accessible entrance is located at the North side of our parking lot.  The clinic can be accessed from the front door or the wheelchair accessible back door.

Do I have to book an appointment?

Appointments need to be booked in advance. However, we do offer same day urgent appointments.

You DO NOT have to book an appointment for the Friday night 5-8 pm and Saturday 10am-3pm after-hours clinics.  Right now due to COVID-19 our Friday night and Saturday hours begin as a Virtual Care telephone appointment.  An In-Person Care appointment can be arranged if that is required for the safety of your care.  Friday night and Saturday appointment times are offered on a first come, first served basis

Do you offer TeleHealth Consultation ?

We currently offer Virtual Care telephone and video conference appointments. However, this is subject to change if OHIP ceases to cover this service after the COVID-19 pandemic.

How do I access my TeleHealth Consultation ?
We run our TeleHealth visits over the secure OTNHub ( Ontario Telemedicine Network)
Desktop/laptop users: Your doctor will send you an e-mail link at the time of your scheduled appointment. You will join your appointment using your web browser. All you need to do is click on the link and your e-visit will start. For the best experience, use the Chrome browser.*
Mobile users: Install the “Pexip Infinity Connect” app for iPhone/iPad or for Android phone/tablet.  Close the app once the installation is complete. Your doctor will send you an e-mail link at the time of your scheduled appointment, click on start e-visit and the App will automatically load, ensure you click on the video button in the app to start the e-visit.
How do I book an appointment?

To book an appointment with one of our physicians you can visit our clinic, send us an email here with your booking details or call the clinic and speak to one of our medical office assistants.

I would like to register as a new patient, what do I do now ?

We ask that you fill in and complete the online patient intake form , you are also kindly requested to complete, sign, and email back the Ministry of health Decleration form which you can download from here. 

Please be ready to provide your Health Card during your first visit.

How do I transfer my medical records to your clinic?

At the conclusion of your first visit, you will complete a transfer record form that allows us to receive your records directly from your old clinic. Please be aware that your old clinic may charge for this process.

What happens at my first appointment?

At your first appointment you will be greeted and asked to fill in any  any pertinent forms  (if you have not already completed them).

During your physician appointment, the physician will speak with you to better understand your medical needs and find out about your life – the patient-physician partnership is very important, and we work to cultivate this from the first visit to ensure we are adequately meeting your needs.

What if I need to see a specialist ?

We have an extensive network of medical specialists who we refer patients to as needed. Your physician will discuss with you if it is necessary to see a given specialist and will organize the referral process.
